About the Role

We are seeking a highly skilled and compassionate Adult SMI Case Manager to join our team at Jewish Family & Children's Service. As a key member of our Behavioral Health team, you will provide critical support to individuals with Serious Mental Illness (SMI) and their families.

Key Responsibilities
  • Community Outreach and Engagement: Develop and maintain relationships with community partners, healthcare providers, and other stakeholders to ensure seamless coordination of care.
  • Case Management and Coordination: Provide ongoing case management services, including assessment, planning, and implementation of individualized care plans.
  • Service Coordination: Facilitate access to behavioral health services, including therapy, medication management, and support groups.
  • Communication and Collaboration: Maintain effective communication with clinical team members, healthcare providers, and other stakeholders to ensure comprehensive care.
  • Transition to Adulthood: Ensure successful transition of youth with SMI to adult services, including coordination of care and support.
  • Patient Rights and Advocacy: Educate patients and families on their rights, including access to treatment, appeals, and complaint processes.
  • Administrative Tasks: Perform administrative duties, including documentation, data entry, and reporting, in accordance with agency procedures.
Requirements
  • Education: Minimum High School Diploma; Bachelor's Degree in a human services field preferred.
  • Experience: Quality experience in the behavioral health field, including working with individuals with SMI.
  • Knowledge and Skills: Working knowledge of common and uncommon mental and behavioral health disorders, including Schizophrenia, Bipolar Disorder, Major Depressive Disorder, PTSD, Personality Disorders, Substance Abuse Disorders, etc.
  • Certifications and Licenses: Ability to acquire a Level 1 Fingerprint Clearance Card, without restrictions; AZ Driver's License and Valid Auto Insurance; MVR matching JFCS driving guidelines.
  • Language Skills: Spanish-English bilingual preferred; Pay Differential offered.
