Adult Case Manager for Seriously Mentally Ill Individuals

We are seeking a dedicated and compassionate Adult Case Manager to join our team and make a meaningful difference in the lives of individuals diagnosed with Serious Mental Illness (SMI). The ideal candidate will possess a strong understanding of mental health disorders, excellent communication skills, and a commitment to providing holistic care.

Responsibilities:
  • Provide ongoing case management services to SMI members receiving behavioral healthcare.
  • Coordinate comprehensive care plans, including behavioral and physical health services.
  • Collaborate with clinical teams, ensuring seamless communication and coordination of care.
  • Facilitate access to resources and support systems for SMI individuals and their families.
  • Advocate for patient rights and navigate appeals and complaint processes.
  • Conduct case management activities in the community, including transportation as needed.
  • Maintain accurate documentation and adhere to agency procedures.
Qualifications:
  • Minimum High School Diploma; Bachelor's Degree in a human services field preferred.
  • Working knowledge of common and uncommon mental & behavioral health disorders.
  • Ability to obtain a Level 1 Fingerprint Clearance Card.
  • Valid Arizona Driver's License and Auto Insurance.
  • Spanish-English bilingual skills are highly desirable.
