POSITION SUMMARY

The Patient Care Coordinator plays a pivotal role in managing the healthcare journey of all patients within our organization. Acting as the primary liaison for the clinical team, this position collaborates closely with a multidisciplinary group to ensure personalized care is delivered effectively. The Coordinator develops tailored care plans for patients enrolled in specific value-based programs, guiding them in adhering to their health strategies to enhance outcomes and satisfaction while minimizing healthcare expenses.

KEY RESPONSIBILITIES

  • Evaluate the medical, social, and psychological requirements of patients.
  • Design individualized care plans and determine the necessary resources for each patient.
  • Educate patients and their families on various health conditions to foster improved health outcomes.
  • Maintain comprehensive documentation within patients' Electronic Health Records.
  • Coordinate with diverse service providers to meet patients' needs effectively.
  • Engage with patients via phone, adhering to internal protocols, to offer suitable care recommendations.
  • Collaborate with healthcare providers to plan routine or urgent visits, ensuring all necessary information is gathered beforehand.
  • Review external clinical data, such as lab results and consultation reports, and assign tasks to appropriate team members.
  • Facilitate patient-specific care coordination with nursing facilities, home health agencies, and hospitals as required.
  • Conduct follow-up calls to ensure continuity of care and assist in scheduling appointments.
  • Delegate responsibilities appropriately to LPNs, MAs, and other support staff.

QUALIFICATIONS

  • Associate Degree in Nursing (ADN) is required; Bachelor of Science in Nursing (BSN) is preferred.
  • Active Nursing License in the State of Nebraska is mandatory.
  • Current BLS/CPR certification is required.
  • A minimum of two years of nursing experience is essential.
  • Preferred qualifications include a BSN and two years of experience in case/care management with adult patients, as well as one year in a triage nursing environment.

WORK ENVIRONMENT AND PHYSICAL DEMANDS

This role is situated within a healthcare environment, necessitating frequent sitting and computer usage, while allowing for variations in physical activity for comfort.

Physical Requirements:

  • Ability to stand for 10% or more of the workday.
  • Ability to walk for 10% or more of the workday.
  • Ability to sit for 80% or more of the workday.
  • Occasional lifting of up to 10 pounds.
  • Occasional bending, squatting, kneeling, reaching above shoulder level, and twisting.
  • Frequent use of hands for simple grasping, fine manipulation, and computer tasks.
  • All sensory skills (speech, vision, smell, touch, and hearing) should be corrected to near normal range.
