Compensation: $20 per hour
Work Schedule: Full-time, Monday to Friday (8 AM - 4 PM) Benefits of Employment:
  • Annual pay increase based on performance evaluations after one year of service
  • Three weeks of vacation, one week of sick leave, and twelve holidays for new full-time employees

Key Responsibilities: The Case Manager plays a crucial role in our Day Program Services, focusing on building and nurturing professional relationships with individuals with developmental disabilities on their caseload, as well as collaborating with interdisciplinary teams to ensure seamless service coordination.
  • Work alongside various departments, individuals, families, residences, and funding sources to ensure services align with individual needs and preferences.
  • Conduct assessments and evaluations to determine support levels and assist in support planning, including periodic re-evaluations as necessary.
  • Oversee the implementation and effectiveness of support plans in collaboration with department staff and the interdisciplinary team, providing guidance on required documentation.
  • Engage in planning meetings (team meetings, DHSP, ISP) to advocate for individual needs and contribute to departmental and individual goals.
  • Provide personal care and assistance to individuals based on their unique needs as they work towards their goals, including coverage for group activities as needed.
Qualifications:
  • High School diploma required; a Bachelor's degree in Human Services or a related field is preferred.
  • Minimum of one year of experience supporting adults with developmental disabilities is preferred.
  • A strong passion and commitment to supporting individuals is essential.
  • Valid driver's license, reliable transportation, and a satisfactory driving record are required.
  • Successful completion of company background and reference checks is mandatory.
