Job Summary:

The Nurse Outreach Program Manager will provide nursing care in the community, coordinate a team of nurses and harm reduction staff, and oversee mobile healthcare services at a Federally Qualified Health Center. The program focuses on serving individuals facing barriers to healthcare, including homelessness and substance use.

Key Responsibilities:
  • Provide professional nursing care, including triage, assessment, intervention, and evaluation, to individuals in the community.
  • Deliver skilled interventions, such as wound care, foot care, medication administration, and vital signs, to patients in need.
  • Coordinate provider visits via telehealth or community settings to ensure timely and effective care.
  • Collaborate with the Southside Harm Reduction Services' drop-in center to provide comprehensive care to individuals.
  • Address health concerns with harm reduction strategies, prioritizing patient-centered care.
  • Assess patient needs and support the care team in meeting those needs, ensuring seamless care transitions.
  • Facilitate rapid access to HIV and hepatitis C care, PrEP, and buprenorphine, promoting health equity.
  • Support care plan and medication adherence, empowering patients to manage their health.
  • Lead staff in transition to outreach-based health services, fostering a culture of excellence.
  • Develop policies and procedures for mobile health services, ensuring quality and efficiency.
  • Monitor quality improvement in clinical care and outreach, driving continuous improvement.
  • Supervise clinical outreach staff and manage medication stock, ensuring a safe and effective care environment.
Supervisory Responsibilities:
  • Directly supervise employees within the Outreach Nursing Program, providing guidance and support.
  • Carry out supervisory responsibilities according to organizational policies, promoting a positive work environment.
  • Qualifications:
  • Bachelor's or master's degree in nursing, with a focus on community health and outreach-based nursing.
  • Public health nurse registration preferred, demonstrating expertise in population health.
  • Experience in community health and outreach-based nursing, with a proven track record of success.
  • Program development, management, and evaluation experience, with a focus on quality improvement.
  • Knowledge of harm reduction principles, with a commitment to patient-centered care.
  • Strong communication skills and a commitment to excellence, with the ability to work independently and in teams.
  • Good judgment and organizational skills, with a focus on efficiency and effectiveness.
