About the Role

Lutheran Social Service of Minnesota is seeking a highly motivated and compassionate Care Coordinator to join our team. As a Care Coordinator, you will play a vital role in supporting individuals and families in need, providing guidance and resources to help them achieve their goals.

Key Responsibilities
  • Assessment and Service Planning: Conduct thorough assessments of clients' needs, develop and implement individualized service plans, and coordinate services to meet their unique requirements.
  • Client Support and Advocacy: Provide emotional support, guidance, and advocacy to clients, helping them navigate complex systems and access necessary resources.
  • Collaboration and Communication: Work closely with clients, families, healthcare providers, and other stakeholders to ensure seamless care coordination and effective communication.
  • Documentation and Reporting: Maintain accurate and detailed records of client interactions, service plans, and outcomes, ensuring compliance with regulatory requirements.
Requirements
  • Education: Bachelor's degree in a human services-related field, such as social work, counseling, or psychology.
  • Experience: Minimum one year of experience in a care coordination or social services role, preferably in a healthcare or human services setting.
  • Skills: Excellent communication and interpersonal skills, ability to work independently and as part of a team, strong organizational and time management skills, and proficiency in electronic health records and other software applications.
