About the Role:
Community Care, Inc. is seeking a skilled Care Manager to join our team. As a Care Manager, you will play a vital role in ensuring the health and well-being of our members by providing comprehensive care coordination and quality assurance monitoring of services.
Key Responsibilities:
- Manage an assigned caseload of program members in collaboration with an RN Care Manager
- Complete comprehensive member assessments, member-centered care plans, and functional screens
- Provide ongoing care coordination and quality assurance monitoring of services
- Maintain required documentation in adherence to established standards
Requirements:
- Education: Bachelor's Degree in a Human Services or closely related area required
- Experience: One year of experience with older adults or persons with physical and/or intellectual disabilities in a social service capacity preferred
- Certification: Eligibility for long-term care functional screener certification is required
- Travel: Regular local travel, requiring use of personal vehicle, valid driver's license, and proof of required insurance coverage
