Position Overview:
Lutheran Social Services of Minnesota is currently seeking dedicated Care Coordinators to join our team. This role is focused on the Children's Long Term Support (CLTS) Waiver program, a federally funded initiative designed to assist children with developmental, physical, or severe emotional disabilities.
As a CLTS Care Coordinator, you will serve as the primary contact for families, guiding them in accessing essential services that enable their children to thrive in a safe home and community environment.
Key Responsibilities:
Under supervision, you will engage in the assessment, planning, and execution of service plans for assigned clients. Your duties will include:
- Promoting client independence and personal growth in alignment with their service plans.
- Delivering services through a trauma-informed and family-centered approach.
- Utilizing evidence-based practices as endorsed by the agency.
- Maintaining accurate time reporting and documentation.
Essential Duties:
Your responsibilities will encompass:
- Assessing functional abilities and client needs using required tools.
- Collaborating with clients, families, and service providers to create and implement comprehensive service plans.
- Coordinating and authorizing services in accordance with established plans.
- Facilitating team meetings centered around client and family needs.
- Completing documentation within specified timelines.

Qualifications:
