Position Title: Community Outreach Worker

Salary Range: $43,680 - $45,760 plus benefits

Overview: The Community Outreach Worker plays a vital role in connecting patients, families, and community members with educational and local resources to promote their wellbeing and enhance their quality of life. A primary duty of the Outreach Worker is to build and nurture trusted relationships with patients, families, and residents of the community.
  • Facilitates patient recruitment and retention across various clinic locations and within targeted service areas.
  • Manages the storage, transportation, and maintenance of outreach equipment (e.g., canopies, tables, chairs) for events and activities.
  • Prepares, transports, and distributes outreach and marketing materials to the community and at events.
  • Engages with faith-based organizations, educational institutions, and community-based organizations to foster partnerships and collaborative agreements.
  • Reports all partnership opportunities to the direct supervisor.
  • Keeps an accurate log of outreach activities and maintains appropriate records and inventory in accordance with organizational policies.
  • Communicates effectively and empathetically with individuals from diverse backgrounds.
  • Distributes educational materials and flyers through community engagement activities.
  • Innovates new strategies for marketing and outreach of health center services.
Qualifications:
  • High School Diploma or equivalent required.
  • CPR/First Aid Certification is mandatory.
  • Familiarity with community resources in the local area.
  • Minimum of 1-3 years of experience in a community clinic or Federally Qualified Health Center (FQHC) setting.
  • Valid California Driver's License.
  • Bilingual and bi-literate in Spanish and English is essential.
  • Experience working with patients from socioeconomically disadvantaged backgrounds and those with limited literacy skills.
