Key Responsibilities:

  • Manage and generate referrals from community partners and agency personnel
  • Engage with potential participants and referrals to discuss program enrollment
  • Provide detailed information about the PACE program through various communication channels
  • Ensure that prospective participants are informed about the program and their rights
  • Assess program eligibility for interested individuals
  • Contribute to enrollment meetings by sharing essential non-clinical information
  • Maintain precise records related to intake and enrollment processes
  • Offer supplementary information for thorough evaluations
  • Coordinate initial assessments for prospective participants
  • Maintain ongoing communication with Program Management
  • Comply with regulatory standards while delivering exceptional customer service
  • Suggest and implement enhancements for program effectiveness
  • Perform additional assigned responsibilities as needed

Required Qualifications:

A Bachelor's degree or higher in Social Work; a Master's degree is preferred.

3-5 years of experience in Social Work, ideally with elderly populations.

Excellent communication, organizational skills, and attention to detail.

Ability to prioritize multiple tasks and manage workload effectively.
