Position Overview:

The role of a Residential Case Manager at The Providence Center involves delivering comprehensive clinical case management services aimed at fulfilling program goals and fostering client autonomy. The emphasis is on addressing individual client requirements through a personalized approach, aiding them in reaching optimal levels of self-sufficiency. As a vital member of the team, your responsibilities will include providing case management, supportive counseling, and psycho-social rehabilitation services.

Key Responsibilities:

  • Deliver case management and support to clients
  • Treat clients with dignity and respect
  • Assist in setting personal goals and engage stakeholders in the planning process
  • Provide assistance with essential medical procedures and personal hygiene
  • Promote independence while respecting diverse values and lifestyles

Qualifications:

A minimum of an Associate's degree in Human Services or a related discipline is required, with a Bachelor's degree being preferred. Previous experience in residential settings is advantageous. A valid driver's license and insured vehicle are essential for this position.
