Role Summary

The primary function of the facility-based Advanced Practice Provider (APP) is to deliver Provider-level direct patient care services within the Post-Acute Care environment.

Key Responsibilities
  • Conduct clinical interventions and episodic direct care visits, along with appropriate follow-up evaluations.
  • Provide ongoing assessment and management of chronic, co-morbid conditions as clinically indicated.
  • Assess and treat residents experiencing changes in their health status.
  • Review and manage medications, orders, and diagnostic tests, responding to clinical abnormalities for all patients under their care, as permitted by collaborating physicians.
  • Coordinate care with collaborating physicians to ensure comprehensive medical management.
  • Perform Annual Wellness Visits for all long-term Nursing Facility residents and may conduct Annual History and Physicals as requested.
  • Assist with facility-wide health issues, where appropriate collaborations exist.
  • Collaborate with other healthcare professionals, including Therapists, Specialists, and Social Workers.
  • Maintain communication with families and participate in family conferences when feasible.
  • Fulfill other duties as assigned.
Qualifications
  • Current nurse practitioner or physician assistant license in the state of practice & valid board certification in your specialty.
  • Master's Degree in Nursing or Physician Assistant Studies.
  • Preferred strong leadership, interpersonal, and problem-solving abilities.
  • Excellent written and verbal communication skills are essential.
  • Must demonstrate punctuality, organizational skills, and the ability to work effectively within a team.
