Position Overview
The Hospice Licensed Practical Nurse (LPN) plays a crucial role in the planning, coordination, and delivery of care for hospice patients, adhering to the established care plan. This position provides direct patient care under the supervision of a registered nurse, ensuring that the plan of care is followed, treatments are administered, and collaboration with the interdisciplinary team is maintained to achieve optimal care outcomes.
Key Responsibilities
- Deliver direct patient care as defined by the State Nurse Practice Act, following a patient assessment and an approved care plan initiated by the registered nurse.
- Engage in focused nursing visits and telephonic triage as needed.
- Participate in night and weekend coverage rotations among the nursing team.
- Collaborate with hospice team members regarding patient care and interventions.
- Conduct focused nursing assessments, recognizing signs and symptoms of end-stage medical conditions and implementing nursing and medical interventions as directed by the registered nurse.
- Administer medications and treatments as prescribed, evaluating patient responses and communicating concerns to hospice providers and registered nurses.
- Contribute to the development and modification of individualized patient care plans under the guidance of the registered nurse.
- Provide education and support to patients and caregivers regarding end-of-life care needs.
Organizational Responsibilities
Perform duties in alignment with the organization's mission, vision, and values while supporting a culture of equity, diversity, and inclusion.
Qualifications
- Graduate of an accredited LPN training program.
- At least one year of experience as a licensed practical nurse in home health, hospice, or acute care settings.
- Current, unencumbered LPN license in the State of Oregon.
- Valid driver's license and access to an insured vehicle.
- CPR certification required within six months of hire.
Knowledge, Skills, and Abilities
- Understanding of federal, state, and local regulations, including Hospice Medicare Conditions of Participation and HIPAA privacy rules.
- Strong nursing skills and the ability to counsel in a hospice environment.
- Proficient in electronic medical records systems and general computer applications.
- Excellent interpersonal skills and the ability to manage stress and grief effectively.
Working Conditions
This position may require travel outside of the workplace and may involve exposure to various hazards. Flexibility in hours may be necessary to meet the needs of the role.
