Job DescriptionJob Summary: We are seeking a compassionate and motivated licensed social worker to join our team as a Medical Social Worker - Hospice Care Specialist. As a critical member of our hospice team, you will provide onsite psychosocial assessments and plans of care, identifying emotional, social, financial, and environmental resources to support our patients and their families.
Key Responsibilities:
- Conduct thorough psychosocial assessments to identify patient needs and develop personalized plans of care.
- Provide emotional support and guidance to patients and their families, helping them navigate the end-of-life care process.
- Collaborate with the care team to ensure seamless delivery of care and services.
- Develop and maintain relationships with community resources and organizations to support patient needs.
- Participate in ongoing education and training to stay up-to-date on best practices in hospice care.
Qualifications:
- Master of Social Work degree from a school of social work.
- Current licensure as a Clinical Social Worker (CSWA) or Licensed Master Social Worker (LMSW).
- Excellent problem-solving, critical thinking, and social skills, with the ability to build trusting relationships with patients and care team members.
- Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities.
- Ability to communicate effectively, both verbally and in writing.
- Self-confidence and a positive attitude, with a commitment to goal achievement and patient-centered care.
