Responsibilities:

  • Providing personal care services such as bathing, grooming, and maintaining hygiene.
  • Assisting patients with dressing and undressing.
  • Planning and preparing nutritious meals, including grocery shopping.
  • Supporting patients during meal times.
  • Monitoring and caring for patients' skin to prevent tissue damage.
  • Communicating any changes in the patient's condition to the Case Manager.
