Responsibilities:
- Providing personal care services such as bathing, grooming, and maintaining hygiene.
- Assisting patients with dressing and undressing.
- Planning and preparing nutritious meals, including grocery shopping.
- Supporting patients during meal times.
- Monitoring and caring for patients' skin to prevent tissue damage.
- Communicating any changes in the patient's condition to the Case Manager.
