Summary:
Acts as a patient advocate, liaison, and advisor/educator to assist patients in navigating their healthcare journey.

Responsibilities:
Regularly assesses assigned patients and monitors their progress in relation to the care plan. Effectively communicates the care plan to patients and their families, addressing any concerns or questions they may have.

Provides emotional support, counseling, clinical education, and expert guidance to empower patients and families to engage meaningfully in their healthcare decisions.

Facilitates communication between patients and clinical staff, administration, physicians, managed care organizations, and community resources to optimize care coordination and address barriers to the care plan.

Assists patients and families in resolving financial, psychosocial, functional, and administrative challenges by providing options and referrals to appropriate resources.

Advocates for patients and families as necessary.
Requires critical thinking skills, decisive judgment, and the ability to work independently. Must thrive in a fast-paced environment and take appropriate actions.

Preferred:
Bilingual in Spanish

Requirements:

Graduate of an accredited or state board of nursing approved Registered/Professional Nursing program if less than 3 years of experience.

State licensure as a Registered Nurse (RN).

Basic Life Support-BLS certification through the American Heart Association or the American Red Cross CPR for the Professional Rescuer, with certification valid before the start date.

BLS or CPR certification must remain valid for sixty days post-hire. Minimum of 2 years of nursing experience.
