Job Overview:We are seeking a skilled Patient Advocate Specialist to join our team at Next Move Healthcare. The successful candidate will be responsible for providing comprehensive psychosocial support to dialysis patients, ensuring that their rights, dignity, and overall quality of care are maintained in a safe environment.
- Salary Range: $65,000 - $85,000 per annum, based on experience.
To be considered for this role, you must possess a Master's degree from an accredited school of social work and state licensure (or eligibility). A minimum of one year of clinical social work experience, preferably in an ESRD setting, is required. CPR certification is preferred but not mandatory.
Key Responsibilities:- Provide comprehensive psychosocial assessments for patients upon admission and annually.
- Support patients and families in adjusting to dialysis, educating them on patient rights, responsibilities, facility rules, and policies.
- Assist patients with financial aid, insurance, transportation, and home health care services, while helping improve community resources.
- Participate in patient care planning as part of the Interdisciplinary Team (IDT), ensuring seamless coordination with service providers.
