About the RoleAs a Patient Advocate, you will be responsible for assessing patient needs, developing personalized care plans, and ensuring seamless communication between healthcare providers. Your strong interpersonal skills and ability to empathize with patients will be essential in building trust and promoting positive outcomes.
Key Responsibilities- Assess patient needs and develop individualized care plans
- Communicate effectively with patients, families, and healthcare teams
- Collaborate with multidisciplinary teams to ensure comprehensive patient care
- Unencumbered license to practice as PA or NP in the state of Maine
- Graduate of an accredited nurse practitioner program
- Current American Heart Association Healthcare Provider BLS Certification required
