Role Overview

We are seeking a skilled Patient Advocate Telehealth Specialist to join our team at Medix. This is an exciting opportunity for a dedicated and compassionate individual to make a meaningful difference in the lives of patients.

Job Responsibilities
  • Coordinated Care Management: Deliver high-quality, patient-centered telehealth services that improve health outcomes and promote wellness.
  • Care Planning: Collaborate with multidisciplinary teams to develop and implement effective care plans that address the unique needs of each patient.
  • Advocacy and Support: Provide empathetic and informed support to patients and their families, navigating complex medical conditions and treatment pathways.
  • Case Management: Coordinate acute healthcare episodes, such as hospitalizations and rehabilitation stays, to ensure seamless transitions and optimal care.
Requirements
  • Bachelor of Science in Nursing (BSN) degree required; current CCM Certification a plus.
  • At least 5 years of experience in nursing, preferably in care management or related field.
  • Strong communication and interpersonal skills, with ability to build rapport with patients, families, and healthcare providers.
  • Excellent problem-solving and critical thinking skills, with ability to navigate complex medical conditions and systems.
  • Ability to work independently and collaboratively as part of a dynamic team.
