Job Overview

Position Title: Children's Targeted Case Manager

Qualifications: A Bachelor's degree in Human Services (such as Psychology, Sociology, Social Work, Special Education, or a related field) is required. Candidates must have at least one year of full-time, post-graduate experience working directly with children. A Master's degree in a relevant field may substitute for the required experience.

Working Hours: Standard hours are Monday to Friday, from 8:00 AM to 4:30 PM, with additional hours as necessary.

Reporting Structure: This role reports directly to the KY Impact Program Director and/or Lead Service Coordinator.

Role Summary: The Children's Targeted Case Manager is responsible for providing targeted interventions to clients associated with Cumberland River Behavioral Health, Inc. The primary goal is to assist children and their families in accessing necessary medical, social, educational, and support services.

Key Responsibilities:

  • Conduct comprehensive needs assessments in collaboration with the child, their family, and relevant stakeholders to identify the child's needs.
  • Assist in developing treatment plans based on assessment data and professional standards.
  • Monitor the child's progress in accessing required services based on the assessment and treatment strategies.
  • Report any instances of abuse (physical, emotional, sexual), neglect, or exploitation as mandated by law.
  • Perform additional duties as assigned.

Performance Expectations:

  • Manage an average caseload of 20-25 billable cases per month as determined by agency standards.
  • Achieve a performance rating of 3 or higher on evaluations; a Performance Improvement Plan will be initiated for ratings below this threshold.
  • Educate families or custodians on the importance of early intervention services and treatment programs.
  • Deliver care in accordance with ethical standards and professional practices.
  • Ensure the accuracy and completeness of client documentation.
  • Provide services in alignment with agency policies and client treatment plans.
  • Advocate for clients effectively.
  • Respond promptly to emergency situations and provide crisis assistance as needed.
  • Collaborate effectively with multidisciplinary team members and work independently when required.

Required Skills:

  • Complete training requirements within specified timeframes.
  • Possess a valid driver's license with a clean driving record.
  • Pass background checks.
  • Hold a valid Social Security number or work permit if not a U.S. citizen.
  • Complete TB Risk Assessment within 7 days of hire.
  • Pass a drug test.
  • Exhibit strong oral and written communication skills.
  • Demonstrate interpersonal skills and the ability to exercise good judgment in various situations.
  • Maintain effective working relationships with diverse individuals.
  • Understand and apply Behavioral Health concepts in practice.

Physical Demands:

  • Ability to perform physical tasks such as reaching, pushing, pulling, stooping, bending, standing, walking, and lifting up to 20 pounds.
  • Obtain CPR/First Aid certification within one month of employment and maintain it.
  • Maintain a valid driver's license and safe driving record if required for job performance.

Work Environment:

The work environment may include outdoor conditions and off-site locations. The noise level is typically mild to moderate.

Job Hazards:

  • Work hours may vary and include flexible scheduling based on organizational needs.
  • Potential exposure to blood or infectious materials may occur during routine tasks.
  • Participation in annual training on infection control and safety is mandatory.
  • New staff must complete training on managing physical and verbal threats, including when to involve law enforcement.
  • CPR certification is required within one month of employment for those providing direct care.
