Position Title:

Children's Targeted Case Manager

Qualifications Required:
  • A Bachelor's degree in Human Services or a related field such as Psychology, Sociology, Social Work, or Special Education is mandatory.
  • At least one year of full-time, post-graduate experience directly working with children is essential.
  • A Master's degree in human services or behavioral science may be accepted in lieu of the required year of experience.
Working Hours:

Standard hours are Monday to Friday from 8:00 AM to 4:30 PM, with additional hours as necessary.

This role reports to the KY Impact Program Director and/or Lead Service Coordinator.

Job Overview:

The Children's Targeted Case Manager plays a vital role in helping children and their families navigate and access essential medical, social, educational, and supportive services.

Key Responsibilities:
  • Conduct thorough needs assessments in partnership with children and their families.
  • Assist in formulating treatment plans grounded in assessment findings and professional guidelines.
  • Track progress in obtaining necessary services and treatment options.
  • Report any instances of abuse, neglect, or exploitation.
  • Provide crisis intervention and advocacy for clients.
  • Maintain professionalism and adhere to ethical standards in all care provided.
Performance Expectations:
  • Manage an average caseload of 20-25 billable cases monthly.
  • Educate families on the importance of early intervention services.
  • Ensure the accuracy and completeness of client documentation.
  • Deliver services in accordance with agency policies and procedures.
  • Collaborate effectively with a diverse range of individuals.
Required Skills:
  • Complete training requirements punctually.
  • Hold a valid driver's license with a clean driving record.
  • Pass background checks and drug screenings.
  • Exhibit strong communication and interpersonal skills.
  • Demonstrate sound judgment and problem-solving abilities.
Additional Requirements:

Must be physically capable of performing job functions, including reaching, pushing, and lifting.

CPR/First Aid certification is required, and a safe driving record must be maintained.

Work Environment:

Occasional work outside the standard office setting may be required, with mild to moderate noise levels.

Health and Safety Considerations:

May involve exposure to blood or infectious materials.

Participation in annual training on safety protocols and infection control is mandatory.

Certification in Cardiopulmonary Resuscitation must be obtained within one month of employment.
