Job Title: Service Coordinator

At Thresholds, we are seeking a compassionate and skilled Service Coordinator to join our team. As a Service Coordinator, you will play a vital role in creating partnerships with our clients as they transition from resource and housing scarcity to living with dignity in a home of their own.

Key Responsibilities:

  • Coordinate mental health services at Thresholds, linking clients to other providers as needed.
  • Build short-term yet impactful rapport with new people, gathering information, providing case management services, and completing assessments.
  • Seek to understand clients' experiences from their perspective and lower barriers to care by linking clients to appropriate services, including medical care, housing, and other resources.
  • Collaborate with the Thresholds team to provide comprehensive support to clients.

Requirements:

  • Passion for mental health advocacy and a commitment to creating a supportive environment for clients.
  • Effective communication and relationship-building skills, with the ability to work with diverse populations.
  • Experience working in mental health, with a focus on housing and case management.
  • Strong time management and organization skills, with the ability to prioritize multiple tasks and deadlines.
  • Proficiency with Microsoft 365 and similar software systems.
  • Valid driver's license, current car insurance, and daily access to your own car.

What We Offer:

  • Competitive pay, with a base rate of $52,000 annually, subject to increase based on education and experience.
  • Generous PTO, including 9 federal holidays, 8 days of sick leave, and 15-22 days of personal and vacation time.
