As a Primary Care APRN Eldron, you will be responsible for providing high-quality, patient-centered care to a dedicated patient population. You will work collaboratively with the medical director(s) to develop clinical protocols and procedures for the delivery of outpatient care. Your expertise will be essential in promoting staff and community enhancement through coordination of in-services and training courses. You will also be responsible for ensuring a clean and safe environment for patients and facilitating the development and implementation of standards of practice, policies, and procedures to enhance care delivery.
Key Responsibilities:
• Conduct comprehensive health assessments, including basic physical examinations, and interpret diagnostic results to determine accurate differential diagnoses.
• Develop and implement formal and informal teaching plans for patients, families, staff, and community, promoting patient/family-informed decision making in treatment plans and options.
• Participate in departmental CQI efforts and provide support for improvement of identified issues, ensuring timely completion of charting and billable assignments.
• Collaborate with staff to identify, mediate, and resolve issues and team problems, promoting a positive and productive work environment.
Requirements:
• Advanced degree in nursing or related field, with certification as an APRN.
• Strong clinical skills and experience in primary care, with ability to work independently and collaboratively as part of a team.
• Excellent communication and interpersonal skills, with ability to promote patient-centered care and staff development.
• Ability to work in a fast-paced environment, with strong organizational and time management skills.
