CenterWell Senior Primary Care Opportunity
We are seeking a Board Certified/Board Eligible Advanced Practice Provider to join our team in Orlando, Florida, as an outpatient geriatric primary care provider. Our unique care model focuses on addressing patient needs and understanding the impact of social determinants of health on health outcomes.
A Day in the Life:
- Longer appointments to spend quality time with patients
- Work in a value-based care model to improve patient quality of life
- See a variety of cases, from preventative care to chronic conditions
- Spend 100% of your time clinically focused on direct patient care
Your Qualifications:
- Active, unrestricted NP/PA license in the appropriate state
- 2-5 Years Primary Care Experience with Chronic Disease Management
- Current certification as an NP or PA
- Current DEA license or ability to obtain a DEA license
- Passionate about contributing to an organization focused on continuously improving consumer experiences
Preferred Qualifications:
- Familiarity with Value-Based Care model
- Familiarity with HEDIS measures
