About the Role

We are seeking a highly skilled and experienced Clinical Manager to join our team at Illumia. This is a key leadership position responsible for overseeing the day-to-day operations of our Advanced Illness Management (AIM) and Palliative Care department.

Key Responsibilities
  • Coordinate and manage all business affairs and clinical activities related to the administration of the AIM and Palliative Care department.
  • Ensure clinical care delivery goals are met and exceeded.
  • Manage a team of Clinical Coordinators, NPs, and RNs.
  • Oversee scheduling, frequency adherence, clinical coordination, medical record maintenance, and marketing functions for a team of patients and clinicians.
  • Manage incoming calls from partners and patients.
  • Ensure Visit Frequency compliance, scheduling PRN visits, post-hospitalization follow-up with patient/family, and expediting discharge home with AIM communication with partners.
  • Order labs, DME, and imaging.
  • Coordinate results with NPs, enter telephone encounters in partner EMR, and enter routine visit med reconciliation in partner EMR.
  • Ensure transitions to HSP referrals to CTC for care type.
  • Assist in identifying HSP eligible patients.
  • Identify HH providers that can accept HH referrals from PCC.
  • Identify and communicate with local partner office for lab drop-off.
Requirements
  • Registered Nurse with Home Health, Hospice, and/or Palliative Care experience preferred.
  • Bachelor's degree in nursing, Business Administration, or related field; or equivalent experience and knowledge in Health Care Operations required.
  • Master's degree in business or related field preferred.
  • Minimum of 2 years health care administration/operations management required.
  • Must be able to work some nights/weekends on call.
  • Certified in Hospice and Palliative Care - preferred.
