Job Summary:
The Training Director for the Transitional Care Management (TCM) program will develop and implement a comprehensive training and workforce development program across our 11-hospital system, ensuring TCM staff are equipped to deliver high-quality transitional care services.
Key Responsibilities:
- Design and direct a systemwide training program for TCM staff, aligning with organizational goals and best practices
- Collaborate with internal and external stakeholders to develop training curricula and operational guides
- Develop ongoing professional development programming for TCM staff, including coaching initiatives
- Conduct workforce planning and analysis to determine TCM staff needs and shortages
- Support the roll-out of standardized TCM programs across facilities
- Partner with Central Office departments and external agencies to develop and integrate TCM training programs
- Monitor and evaluate training effectiveness, recommending priorities for training resources
- Develop and direct wellness initiatives for TCM staff
- Foster collaboration with the Epic team to optimize EHR functionalities supporting TCM workflows
- Utilize data analytics to monitor key performance indicators related to training outcomes
Requirements:
- Baccalaureate Degree in Education, Public Health, Psychology, Personnel Administration, Management, or a related field
- Eight years of experience in planning and manpower development, medical or public health administration, or related fields, with at least three years in a managerial or supervisory capacity
- Extensive knowledge of training and manpower development principles and ability to analyze organizational needs
