POSITION SUMMARY:

The Community Support Coordinator plays a vital role in assisting individuals facing significant mental health challenges. This position involves a range of responsibilities aimed at enhancing the well-being of clients through tailored support and resource management.

KEY RESPONSIBILITIES:

  • Formulate personalized support strategies for individuals experiencing severe mental health issues.
  • Serve as a bridge between clients and various service providers to ensure comprehensive care.
  • Execute support strategies effectively to help clients reach their personal objectives.
  • Keep precise and up-to-date records of client interactions and progress.
  • Facilitate transportation for clients when required to access services.
  • Fulfill necessary administrative tasks to support program operations.

ESSENTIAL QUALIFICATIONS:

  • A bachelor's degree in social work, psychology, or a related discipline.
  • Prior experience in mental health services, case management, or community outreach is advantageous.
  • A valid driver's license and access to a dependable vehicle.
  • Familiarity with the mental health demographic and available community resources.
  • Capability to work independently as well as collaboratively within a team environment.
  • Strong communication and interpersonal abilities are essential.
