Care Coordination

We are seeking a skilled Care Coordinator to join our team at Vibrant Health - Wyandotte Neighborhood Clinics. As a key member of our healthcare delivery model, you will provide coordinated and comprehensive patient outreach and care coordination that follows the Patient-Centered Medical Home model.

Your primary responsibility will be to ensure patient care is coordinated and comprehensive by providing patient outreach, care coordination, education, and follow-up services. This will involve collaborating with healthcare teams to monitor all outgoing referrals, conducting weekly referral tracking and follow-up, and maintaining proper documentation throughout the referral process.

Additionally, you will generate weekly reports tracking the success rate of outgoing referrals and identifying potential problems or barriers to care. You will also monitor outgoing and incoming DI results, working with care teams to retrieve any missing or incomplete results.

Medical Records Administration and Technical/Support Services

As part of our team-based healthcare delivery model, you will assume responsibility for electronic medical records quality control services. This will include monitoring patient demographics, ensuring accurate information is entered into the patient management system, and reviewing patient documents for accuracy of data.

You will also scan and route patient documents in the patient management system, review and distribute faxes received, and request records from outside organizations as needed. Furthermore, you will complete requests for medical records in a timely manner and track responses.

Requirements

To be successful in this role, you will need two years of relevant working experience providing Care Coordination or Patient Service Representative. Bilingual fluency in written and oral English/Spanish is preferred, and proficiency in the use of personal computers and Microsoft Office Suite is required.

You must be able to develop rapport with patients and their families, community, and other healthcare personnel, and exhibit a professional and respectful demeanor. You will also need to work successfully within a team environment and collaborate with other professional staff.

Preferred Qualifications

A working knowledge of electronic health records, medical terminology, and third-party healthcare coverage is preferred. Prior experience working in a pediatric healthcare environment is also desirable.
