Job Description:

Spira Care is seeking a highly skilled and compassionate Care Guide RN to join our team. As a key member of our Care Team, you will play a vital role in delivering exceptional patient experiences and ensuring seamless healthcare navigation.

Key Responsibilities:
  • Establish and maintain trusted partnerships with patients, families, and healthcare providers to ensure comprehensive care coordination.
  • Conduct thorough patient assessments, analyzing clinical, functional, financial, and psychosocial factors to develop and implement individualized care plans.
  • Collaborate with the Care Team to monitor and evaluate the effectiveness of care plans, making adjustments as needed to optimize health outcomes.
  • Provide exceptional customer service, communicating effectively with patients, families, and healthcare providers through various channels.
  • Act as a liaison between patients, employers, providers, and community organizations, fostering continuity and coordination of care.
Requirements:
  • Associate or bachelor's degree in nursing.
  • Valid and active Registered Nurse (RN) license in Kansas and Missouri or multi-state compact license.
  • 3 years of full-time direct patient care and clinical experience in medical/surgical, pediatrics, or obstetrics.
  • Intermediate level knowledge of Clinical Guidelines.
  • Previous experience in customer service or serving customers in some direct capacity.
Preferred Qualifications:
  • Bachelor's degree in nursing.
  • Case Management Certification.
  • 5 years' experience in Care/Case/Disease Management, Health Plan, or Utilization Review.
  • Thorough understanding of advanced primary care and the Patient Centered Medical Home (PCMH) / Primary Care First (PCF) model.
