SUMMARY
The Social Worker plays a pivotal role in embodying the values of Roane General Hospital by serving as an advocate for clients, facilitating access to essential resources, and assisting individuals and families in navigating personal and familial challenges. This position requires the social worker to provide comprehensive support in coordinating care and addressing the unique needs of patients throughout their oncological treatment journey.

The Social Worker is responsible for assessing, planning, implementing, and evaluating individualized mental health care plans, ensuring that quality clinical outcomes are achieved. This role includes managing care for adult patients facing mild to severe medical conditions and oncological diagnoses, as well as overseeing the coordination of services and case management for this demographic.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following:
  • Facilitate patient progression through the healthcare continuum.
  • Identify and resolve systemic issues that may hinder diagnostic or treatment processes.
  • Collaborate with multidisciplinary teams to enhance discharge planning and patient care.
  • Utilize approved screening tools to identify at-risk populations and adhere to reporting protocols.
  • Manage indigent drug coverage and support services for oncology patients.
  • Participate in clinical performance improvement initiatives.
  • Maintain exceptional interpersonal communication skills, both written and verbal.
  • Oversee the oncology resource center and manage donations.

Additionally, the Social Worker will provide feedback on clinical documentation, assist in policy development, and address patient complaints while ensuring compliance with regulatory standards. The role demands a commitment to professional development and a proactive approach to learning new policies and procedures.

QUALIFICATIONS
To be successful in this role, candidates must possess a current social work license and demonstrate the ability to analyze and interpret complex information. Strong mathematical skills and reasoning abilities are essential for effective problem-solving and decision-making.

PHYSICAL DEMANDS
This position requires the ability to perform various physical tasks, including lifting and moving patients, and may involve exposure to hazardous materials. Adherence to safety protocols is mandatory to maintain a safe working environment.
