Summary:
The Social Worker plays a crucial role in supporting the values of Roane General Hospital by advocating for clients to access necessary resources and aiding them in navigating personal and familial challenges. This position involves assisting clients and their families in coordinating care and addressing individual needs throughout the oncological treatment journey.

The Social Worker is responsible for assessing, planning, implementing, and evaluating the mental health care plan of patients, ensuring quality clinical outcomes are achieved. This includes managing care for adult patients with varying degrees of medical conditions and oncological diagnoses, as well as overseeing service coordination and case management for this demographic.

Key Responsibilities:
  • Facilitates patient progression through the healthcare continuum.
  • Identifies and resolves systemic issues that may hinder diagnostic or treatment progress.
  • Collaborates with multidisciplinary teams during the discharge planning process.
  • Coordinates indigent drug coverage and support services for oncology patients.
  • Participates in clinical performance improvement initiatives.
  • Maintains exceptional interpersonal communication skills.
  • Oversees the oncology resource center and manages donations.

Qualifications:
