SUMMARY
The Social Worker will embody the values of Roane General Hospital by serving as an advocate, assisting clients in accessing essential resources and managing personal and familial challenges. This role requires the provision of support to clients and families in navigating care coordination and addressing individual needs throughout the oncological treatment journey. The Social Worker must be adept at assessing, planning, implementing, and evaluating the mental health care plan to ensure quality clinical outcomes.

Key responsibilities include:
  • Providing care management and coordination for adult patients facing mild to severe medical conditions and oncological diagnoses.
  • Overseeing service coordination and case management for this patient population, including managing indigent pharmaceutical coverage.
  • Collaborating with healthcare team members to empower patients in managing their health conditions.
  • Ensuring effective communication and collaboration during all phases of discharge planning.
  • Participating in clinical performance improvement initiatives and utilizing data to enhance quality measures.

ESSENTIAL DUTIES AND RESPONSIBILITIES
Include but are not limited to:
  • Facilitating patient progression through the care continuum.
  • Identifying and resolving system issues that hinder diagnostic or treatment progress.
  • Consulting with relevant disciplines to expedite care and discharge.
  • Identifying at-risk populations using approved screening tools.
  • Coordinating support services and managing indigent drug coverage for oncology patients.
  • Actively engaging in clinical performance improvement activities.
  • Maintaining high standards of patient care and customer service.

QUALIFICATIONS
To successfully perform this role, candidates must possess:
  • A current West Virginia social work license.
  • Exceptional interpersonal and communication skills.
  • The ability to analyze and interpret various documents and reports.
  • Strong reasoning and critical thinking abilities.

WORK ENVIRONMENT
The work environment may include exposure to moving mechanical parts, fumes, and potential hazards associated with patient care. Adherence to safety policies and procedures is essential to maintain a safe working environment.
