Healthcare Case Coordinator (RN)

Position Overview:

Millennium Physician Group is on the lookout for a dedicated full-time Healthcare Case Coordinator to become a vital part of our ACO team.

We seek an enthusiastic and organized individual who excels in multitasking and possesses innovative thinking skills.

Key Responsibilities:

The Healthcare Case Coordinator will provide essential services that help patients achieve optimal wellness.

This role emphasizes client-centered care, linking patients and their families with healthcare and community resources to guarantee timely and coordinated access to appropriate medical and support services, ensuring continuity of care.

Responsibilities include assessment, planning, coordination, advocacy, education, monitoring, and maintaining accurate documentation across all program areas.

The Coordinator must demonstrate an understanding of growth and development and have the ability to assess, communicate, and care for diverse patient populations.


Compensation and Benefits:

This position offers competitive pay along with a comprehensive benefits package, including a 401(k) plan with matching contributions and three weeks of paid time off. Qualifications:

Applicants must be a Registered Nurse with an active and clear license in the State of Florida, which must be maintained throughout employment. A minimum of three years of clinical experience, including at least one year in case management, is preferred.

Skills and Competencies:

To excel in this role, candidates should possess strong computer skills, particularly in Excel, and familiarity with EMR systems; knowledge of Athena is a plus but not mandatory.
