Job Title: Registered Nurse - Case Manager

**Job Summary:**

We are seeking a skilled and compassionate Registered Nurse - Case Manager to coordinate patient care and provide holistic medical management services to beneficiaries with complex health needs.

**Key Responsibilities:**

  • Coordinate patient care with healthcare professionals to achieve optimal outcomes in clinical care, quality, and cost-effectiveness.
  • Ensure compliance with standards of care and practice in accordance with established policies, procedures, and guidelines.
  • Perform physical exams, health histories, and provide health promotions, counseling, and education.
  • Administer medications, wound care, and other personalized interventions as needed.
  • Direct and supervise care provided by other healthcare professionals.
  • Accountable for making patient care assignments based on scope of practice and skill level of assigned personnel.
  • Recognize adverse signs and symptoms and react quickly in emergency situations.
  • Communicate and collaborate with diverse groups of people to inform the healthcare team of plans and actions.
  • Make referral appointments and arrange specialty care as appropriate.
  • Perform nursing services identified in the task order.
  • Conduct research to support improved practice and patient outcomes.

**Additional Responsibilities:**

  • Perform a full range of RN duties in accordance with assignment under the task order, including assessment, planning, interventions, and evaluations.
  • Provide comprehensive medical management services to beneficiaries with catastrophic, serious, chronic, and/or high-risk illnesses or conditions.
  • Participate in multidisciplinary teams with other healthcare providers to provide consultative services and optimal healthcare delivery to patients.
  • Support patient self-management of patient care with external healthcare organizations and facilities.
  • Manage high-risk patient care, including patients with multiple comorbidities or high-risk readmission to a hospital setting.
  • Assist patients with care coordination, including referral issues and communication with the patient using secure messaging.
  • Perform timely, accurate, and concise documentation of patient admission documentation and individualized case management care plans.
  • Participate in on-site designated Extension for Community Healthcare Outcomes (ECHO) tele-mentoring training program.
  • Participate in tele-mentoring, tele-consults, and tele-health to include care coordination/training.

**Requirements:**

  • Possess a Baccalaureate Degree in Nursing, a Graduate Diploma in Nursing, or an Associate Degree in Nursing from an accredited agency.
  • Possess one year of clinical work experience as a full-time RNCM within the preceding three years working in a medical facility.
  • Possess and maintain a current unrestricted license to practice as a registered nurse in any one of the 50 States, the District of Columbia, the Commonwealth of Puerto Rico, Guam, or the U.S. Virgin Islands.
  • Provide two letters of recommendation written within the last two years attesting to the HCW's clinical skills and abilities.
