Job Overview

POSITION SUMMARY

This is a full-time role focused on delivering Case Management services aimed at facilitating the Coordinated Entry process for clients. The ideal candidate will demonstrate a strong commitment to serving the most vulnerable populations, including homeless veterans and families facing homelessness in the local area. KEY RESPONSIBILITIES

  • Support and uphold the philosophy of Catholic ministry and the mission of the Diocese of Austin in both professional and personal conduct.
  • Serve as a model of Gospel values by exemplifying the teachings of the Catholic Church.
  • Engage in continuous professional development through participation in religious ceremonies, training sessions, and reflective practices.
  • Conduct coordinated entry screenings and needs assessments for veterans and their families to determine eligibility for programs.
  • Work collaboratively with clients through Case Management services to create, implement, and monitor plans for achieving permanent housing.
  • Perform street outreach to identify veterans and families experiencing homelessness.
  • Oversee all aspects of service delivery, including intake, assessment, housing stabilization recommendations, treatment planning, budgeting, case conferences, referrals for additional services, family meetings, mental health and substance use counseling, advocacy, and coordination with community partners.
  • Ensure compliance with grant guidelines through participant recertification.
  • Maintain accurate and timely documentation of service delivery while ensuring client confidentiality.
  • Input veteran and family data into the HMIS, tracking progress and outcomes.
  • Document intake, contact, and temporary financial assistance.
  • Facilitate necessary paperwork for third-party vendors regarding temporary financial assistance.
  • Conduct thorough assessments to gather financial, environmental, psychosocial, employment, educational, housing, and health information to develop Individual Service Plans.
  • Ensure adherence to the agency's fiscal and billing procedures.
  • Participate in all relevant staff meetings, supervision, and training sessions.
  • Complete required documentation to meet funding, licensing, operational, and quality assurance standards.
  • Prepare and submit all necessary reports and paperwork accurately and promptly.
  • Maintain confidential client files, statistical records, and case notes.
  • Perform additional duties as assigned by management.
  • Actively contribute as a team member within Catholic Charities.
  • Develop and sustain a comprehensive understanding of Catholic Charities programs and community resources.
  • Maintain a work schedule that optimizes availability to clients and colleagues.
  • Report directly to the management of Veteran Services.

QUALIFICATIONS

  • Education: Bachelor's degree from an accredited institution or equivalent experience.

  • Experience: Relevant work experience may substitute for educational requirements at a ratio of two years of experience for one year of education.

  • Licenses/Certifications: Valid driver's license required. Compliance with the Diocese of Austin Ethics and Integrity in Ministry policies is mandatory throughout employment.

PREFERRED QUALIFICATIONS

  • Veteran or family member of a veteran/service member of the U.S. Armed Forces.

LANGUAGE REQUIREMENT

  • Proficiency in English, including speaking, reading, and writing.
