Job Overview

Position Summary: We are seeking a dedicated and enthusiastic individual to deliver comprehensive case management services to residents while designing innovative programs that assist them in successfully maintaining their housing, accessing mental and primary health care, managing finances, and pursuing educational and employment opportunities. An ideal candidate will provide holistic case management and cultivate a positive community atmosphere within our permanent supportive housing environments for individuals who have previously experienced homelessness or are very low-income.

Key Responsibilities:

  • Implement intensive case management and care coordination to enhance residents' access to and engagement in mental and primary health care, substance use treatment, financial management, and overall self-sufficiency through a collaborative team approach. Work closely with partner organizations and the broader Resident Services team to improve residents' housing stability, income security, health and wellness, and social connections.
  • Effectively assess and manage crisis situations using de-escalation techniques, contacting appropriate authorities in emergencies, and thoroughly documenting incidents and outcomes as necessary.
  • Develop innovative strategies to build rapport with residents, providing proactive outreach to identify needs, enhance residents' involvement in their treatment, and connect them with suitable resources. Create voluntary groups, classes, outings, and unique learning experiences that promote positivity, growth, community engagement, and socialization for residents.
  • Collaborate with Property Management under a blended management model. Regularly meet with Property Management staff to discuss policy enforcement and amendments, collaboratively address challenging situations with residents, and make joint decisions to ensure the safety and well-being of the community.
  • Document services provided in compliance with grant requirements and internal documentation systems. When applicable, submit documentation by established deadlines to the appropriate personnel.


Minimum Qualifications:

  • A minimum of one year of experience in delivering intensive case management services to adults facing homelessness, transitioning from homelessness, or at risk of homelessness is required.
  • At least one year of experience in mental health/substance use and crisis intervention is required.
  • Must be certified in SSI/SSDI Outreach, Access, and Recovery (SOAR), or obtain certification within the first 90 days of employment.
  • Ability to work independently, solve problems, think critically, and communicate effectively with individuals from diverse backgrounds is essential.
  • Strong interpersonal skills, effective organizational methods, and the ability to multitask are required. Must be able to build warm, professional relationships with residents by demonstrating respect, kindness, and availability, as well as a person-centered, strengths-based approach in case management.


Preferred Qualifications:

  • A Master of Social Work Degree (or related field) is recommended.
  • Proficiency in Spanish or American Sign Language (ASL) is advantageous.


Working Conditions/Physical Requirements: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this role. Compensation: $55,500 annually. This position profile outlines the key responsibilities and performance expectations. It cannot encompass all specific job tasks that an employee may be required to perform. Employees are expected to follow any other job-related instructions and perform job-related duties as reasonably assigned by their supervisor.
