Key Responsibilities:

  • Develop and implement Person-Centered Plans/Individual Support Plans for clients
  • Organize and coordinate engaging activities for clients within our program and in the community
  • Collaborate with the Program Specialist or Program Specialist/Site Supervisor to ensure seamless service delivery
  • Maintain accurate records and reports as required

Requirements:
