The Child & Family Intake Coordinator is responsible for conducting screening and intake interviews with children and their families facing significant emotional challenges. This role involves collaboration with psychiatric and allied health professionals in developing treatment plans, interventions, and managing crisis situations. The coordinator will handle initial documentation and oversee case assignments.

KEY RESPONSIBILITIES include, but are not limited to:

1. Conducting initial intake evaluations as required.

A.

Completing thorough integrated bio-psychosocial assessments and necessary admission paperwork within specified timeframes.

B.

Arriving punctually as scheduled and at the designated location.

C.

Exhibiting the ability to assess resource and service requirements comprehensively.

D.

Determining and assigning appropriate cases/programs based on medical necessity.

E.

F.

Completing level of care evaluations within required timelines.

Collaborating with the ACCESS Center for member enrollment.

2. Facilitating individual and family-centered treatment planning tailored to client needs.

A.

Assisting in the development of written individual service plans, reviews, and case closure summaries.

Implementing Person-Centered Planning principles.

2.

Integrating assessments into service planning to establish meaningful individual goals and objectives.

3.

Regularly reviewing treatment plans and adjusting goals and objectives as necessary.

4.

Documenting progress notes that are goal-oriented and reflect the strengths and needs of the client and family.

B.

Ensuring timely completion of required documentation.

Reviewing treatment plans with families and obtaining necessary signatures.

3.

Finalizing progress notes after each interaction.

4.

Ensuring treatment plans and reviews are completed within established timelines.

Completing all mandatory training within the organization’s specified timeframes.

3. Effectively managing intakes and crisis interventions.

A.

Possessing knowledge of therapeutic approaches and applying interventions effectively in therapy.

B.

Understanding necessary and available community resources.

C.

Demonstrating the ability to triage clients effectively.

D.

Making service recommendations that align with the client's identified issues.

4. Participating in clinical team meetings and coordinating team interventions.

A.

Developing treatment goals and objectives that reflect the child's strengths and areas for growth.

B.

Consulting with team members as appropriate and integrating their input.

C.

Taking responsibility for assigned crisis coverage.

5. Upholding professional and ethical standards.

A.

Continuing to demonstrate proficiency in professional practice through participation in case conferences, in-services, workshops, and required training.

B.

Maintaining a record free of substantiated recipient rights violations.

6. Direct service provision.

A.

Maintaining a minimum of 85 hours of direct service monthly.

7. Engaging in committee work and undertaking additional tasks as assigned.

8. Completing other assigned duties within a reasonable timeframe while meeting quality expectations.

The Child & Family Intake Coordinator thoroughly assesses client needs across multiple dimensions (behavioral, health, educational, financial, relational, meaningful activities, quality of life, etc.). Based on these assessments, the coordinator applies interventions aimed at reducing challenging behaviors and enhancing family dynamics, communication, and overall quality of life.

QUALIFICATIONS - To succeed in this role, candidates must meet the following requirements:

EDUCATION and/or EXPERIENCE - Required: A Master's degree in Social Work, Counseling, or Psychology from an accredited institution, along with two years of experience working with children and youth experiencing serious emotional disturbances.

LANGUAGE SKILLS - Exceptional verbal and written communication skills. Ability to build professional relationships with all staff and effectively represent the agency in community networking.

CREDENTIALING, LICENSURE - Required: State of Michigan LARA professional licensure in Social Work (LLMSW, LMSW), Counseling (LLC, LPC), or Psychology (LLP).
