The Child & Family Intake Coordinator is responsible for conducting screening and intake interviews with children and their families who are facing significant emotional challenges (SEC), are at risk of hospitalization, or are vulnerable in various aspects. This role involves collaboration with psychiatric and allied health professionals in treatment planning, intervention, and crisis management. The coordinator is tasked with completing initial documentation and coordinating case assignments.

KEY RESPONSIBILITIES include, but are not limited to:

1. Conducting initial intake evaluations as required.

A.

Completing thorough integrated bio-psychosocial assessments and related admission paperwork within the designated timeframe.

B.

Arriving at work punctually as scheduled and at the assigned location.

C.

Exhibiting the ability to identify resource and service needs comprehensively.

D.

Determining and executing appropriate case/program assignments based on medical necessity.

E.

F.

Completing level of care assessments within required deadlines.

Collaborating with the ACCESS Center within the PIHP for member enrollment.

2. Developing individual/family-centered treatment plans tailored to client needs.

A.

Assisting in or creating written individual service plans, reviews, and case closure summaries.

Implementing Person-Centered Planning principles.

Integrating assessments into service planning to establish meaningful individual goals and objectives.

3.

Regularly reviewing treatment plans and incorporating any necessary additional goals and objectives.

4.

Progress notes are goal-oriented and reflect the strengths and needs of the client and family.

B.

The Coordinator ensures timely completion of required documentation.

Treatment plans and related forms are discussed with the family, and necessary signatures are obtained.

3.

Progress notes are recorded after each interaction.

4.

Treatment plans and reviews are finalized within established timelines.

Completing all mandatory training within the Agency's established timeframes.

3. The intake coordinator effectively manages intakes and crisis interventions.

A.

The coordinator possesses knowledge of treatment methodologies and can effectively apply interventions in therapy.

B.

The coordinator is aware of necessary and available community resources.

C.

The coordinator is capable of effectively triaging clients.

D.

Service recommendations align with the client's identified issues.

4. The intake/crisis staff engages in clinical team meetings and coordinates team interventions.

A.

The coordinator can develop treatment goals and objectives that are consistent with the child's strengths and areas needing improvement.

B.

The coordinator consults with other team members when appropriate and integrates their input as necessary.

C.

The coordinator takes responsibility for assigned crisis coverage.

5. The coordinator exemplifies professional and ethical conduct.

A.

The coordinator consistently demonstrates proficiency in their professional practice by attending regular case conferences, in-services, workshops, and required training.

B.

The coordinator has no substantiated violations of recipient rights.

6. Direct service time.

A.

The coordinator maintains a minimum of 85 hours of direct service time monthly.

7. The coordinator participates in committees and takes on additional tasks as assigned.

8. Completes other duties as assigned within a reasonable timeframe while meeting quality expectations.

The Child & Family Intake Coordinator thoroughly assesses client needs across multiple dimensions (behavioral, health, educational, financial security, relationships, meaningful activities, quality of life, etc.). Based on client and family needs, the coordinator applies interventions to mitigate challenging behaviors and family discord, enhance communication, and improve quality of life. QUALIFICATIONS - To perform this role successfully, an individual must be able to fulfill each essential duty satisfactorily. The requirements listed below represent the necessary knowledge, skills, and abilities.

EDUCATION and/or EXPERIENCE – Required: A Master's degree in Social Work, Counseling, or Psychology from an accredited institution, along with two years of experience working with children and youth experiencing serious emotional disturbances.

LANGUAGE SKILLS - Excellent verbal and written communication skills. Ability to establish professional working relationships with all staff and effectively articulate the agency's perspective when networking within the community.

CREDENTIALING, LICENSURE – Required: State of Michigan LARA professional licensure in Social Work LLMSW, LMSW, Counseling LLC, LPC, or Psychology, LLP.
