Position Overview

The Child & Family Wraparound Facilitator plays a crucial role in assisting youth facing significant emotional challenges and their families. This position emphasizes a family-centered, youth-driven planning approach. The Wraparound Facilitator promotes the engagement of all relevant service systems and natural supports in the lives of children and families. Utilizing a collaborative team strategy, the Facilitator listens to and aids the youth and family in recognizing their unique needs. This planning framework offers families a structured method to leverage their strengths and capabilities to address those needs.

Key Responsibilities include, but are not limited to:

  • Completion of the MDHHS 3-Day Wraparound Training within the initial three months of employment.
  • Consistent punctuality and attendance at the designated work location.
  • Fulfillment of all mandatory training requirements within specified timeframes.
  • Facilitation of transportation for clients to community engagements, clinical appointments, and other resources as outlined in their personalized care plan.
  • Certification as a reliable MichiCANS/CAFAS/PECFAS rater through attendance at the required training within the first three months of employment.
  • Participation in annual training sessions, including ongoing education on various topics to ensure high-quality support for families in relation to Wraparound services; attendance at a minimum of one quarterly Wraparound-specific training is required.
  • Management of a caseload comprising up to 10-12 families, plus an additional 3 in transition.
  • Regular weekly communication with the child and family, and/or Child and Family Team members regarding the implementation of the Wraparound Plan.
  • Formation of a Child and Family team within two weeks of enrollment by engaging with the family to identify key members, natural supports, agency representatives, and other significant individuals.
  • Coordination of Child and Family team meetings, guiding participants throughout the Wraparound process while ensuring adherence to model fidelity and Wraparound principles.
  • Collaboration with families to determine the frequency of team meetings, with an initial standard of at least weekly or bi-weekly meetings until the family transitions towards graduation.
  • Assistance in identifying the strengths and needs of the youth and family, addressing any immediate requirements.
  • Completion of the initial strengths assessment and prioritization within 30 days of enrollment.
  • Collaboration with the family to develop a comprehensive Support Plan, including proactive and reactive crisis/safety strategies, within one week of enrollment.
  • Submission of an Individualized Plan of Care, detailing community-based and culturally relevant services and resources within 30-45 days of enrollment, with subsequent reviews every 90 days.
  • Ensuring that the needs and services identified by the Child and Family team are adequately met.
  • Proactive engagement with community resources, supported by the Child and Family Team and Community Team.
  • Collaboration with other professionals involved in the youth and family’s care, maintaining regular communication and inviting them to team meetings with appropriate notice.
  • Addressing any barriers to the implementation of the Plan.
  • Informing families about relevant community activities and encouraging their involvement in leadership and advocacy.
  • Adherence to Person-Centered Planning principles.
  • Integration of assessments into service planning to establish meaningful and individualized goals and objectives.
  • Timely completion of necessary documentation and assessments, ensuring all required information is included.
  • Submission of progress notes within 48 business hours following service delivery.
  • Management of service plans and reviews within established timelines.
  • Maintaining a clean record with no substantiated recipient rights violations.
  • Achieving a minimum of 80 direct service hours, unless otherwise directed by supervisory approval.
  • Participation in committees and additional tasks as assigned.
