The Child & Family Intake Coordinator plays a crucial role in conducting screening and intake evaluations for children and their families facing significant emotional challenges. This professional works collaboratively with psychiatric and allied health team members to develop treatment strategies, intervene in crises, and facilitate effective care coordination. Responsibilities include managing initial documentation and overseeing case assignments.
KEY RESPONSIBILITIES include, but are not limited to:
1. Conducting initial intake evaluations as required.
A.
Completing thorough integrated bio-psychosocial assessments and necessary admission paperwork within designated timeframes.
B.
Arriving at work punctually as scheduled and at the assigned location.
C.
Exhibiting the ability to identify resource and service requirements comprehensively.
D.
Determining and executing appropriate case/program assignments based on medical necessity.
E.
Completing level of care assessments within required timelines.
Collaborating with the ACCESS Center within the PIHP for member enrollment.
2. Delivering individualized and family-centered treatment planning tailored to client needs.
A.
Assisting in the development of written individual service plans, reviews, and case closure summaries.

Integrating assessments into service planning to establish meaningful individual goals and objectives.

Regularly reviewing treatment plans and adjusting goals and objectives as necessary.

Documenting progress notes that are goal-oriented and reflect the strengths and needs of the client and family.

Reviewing treatment plans with families and obtaining necessary signatures.

Finalizing progress notes after each interaction.

Maintaining a minimum of 85 hours of direct service monthly.

8. Completing other assigned duties within a reasonable timeframe while meeting quality expectations.

QUALIFICATIONS - To succeed in this role, candidates must meet the following requirements:
EDUCATION and/or EXPERIENCE - Required: A Master's degree in Social Work, Counseling, or Psychology from an accredited institution, along with two years of experience working with children and youth experiencing serious emotional disturbances.
LANGUAGE SKILLS - Exceptional verbal and written communication skills. Ability to establish professional relationships with all staff and effectively represent the agency in community networking.
CREDENTIALING, LICENSURE - Required: State of Michigan LARA professional licensure in Social Work (LLMSW, LMSW), Counseling (LLC, LPC), or Psychology (LLP).
