Job Summary

The Patient Navigator plays a vital role in ensuring seamless patient care at Osborn Family Health Center. This position serves as a liaison between patients, providers, and care team members, promoting effective communication and coordination of care.

Key Responsibilities
  1. Provide exceptional customer service to patients, addressing their needs and concerns in a timely and professional manner.
  2. Maintain accurate and up-to-date patient demographic information, ensuring seamless communication and care coordination.
  3. Ensure strict compliance with HIPAA regulations, safeguarding patient confidentiality and privacy.
  4. Fluently communicate with patients in English and Spanish, facilitating language access and reducing barriers to care.
  5. Assist patients with language barriers, providing translation services and support as needed.
  6. Coordinate patient-centered care, working collaboratively with healthcare providers, hospital facilities, and other healthcare entities.
  7. Track and monitor provider orders for outpatient services, ensuring timely and accurate communication.
  8. Attach results and consult reports for provider review, facilitating informed decision-making.
  9. Improve patient compliance through proactive communication, written correspondence, and the patient portal.
  10. Assist patients with lost or misplaced orders, ensuring timely resolution and minimizing delays.
  11. Notify ordering providers of patient non-compliance, promoting timely intervention and care coordination.
  12. Facilitate follow-up appointments as needed, ensuring seamless care transitions.
  13. Engage in ongoing training and professional development, staying up-to-date on best practices and industry developments.
  14. Communicate effectively with colleagues, promoting a collaborative and supportive work environment.
  15. Make recommendations to improve departmental efficiency, contributing to the organization's continuous improvement.
  16. Work effectively under pressure, prioritizing tasks and managing competing demands.
  17. Cooperate with colleagues to cover staff members and provide support as needed.
  18. Ensure compliance with all company policies, procedures, and guidelines, upholding the organization's standards and values.
  19. Undertake special projects and administrative duties as needed, contributing to the organization's overall efficiency and effectiveness.
  20. Communicate concerns and patient issues with the Director of Nursing and/or Risk/Claims Manager, ensuring timely resolution and follow-up.
Requirements

At least one year of experience as a Certified Medical Assistant or equivalent combination of education and experience. Strong interpersonal and communication skills, with the ability to work effectively in a fast-paced environment. Fluency in English and Spanish, with the ability to communicate with patients and providers in both languages.
