The Patient Navigator plays a vital role in ensuring seamless patient care at Osborn Family Health Center. This position serves as a liaison between patients, providers, and care team members, promoting effective communication and coordination of care.
Key Responsibilities- Provide exceptional customer service to patients, addressing their needs and concerns in a timely and professional manner.
- Maintain accurate and up-to-date patient demographic information, ensuring seamless communication and care coordination.
- Ensure strict compliance with HIPAA regulations, safeguarding patient confidentiality and privacy.
- Fluently communicate with patients in English and Spanish, facilitating language access and reducing barriers to care.
- Assist patients with language barriers, providing translation services and support as needed.
- Coordinate patient-centered care, working collaboratively with healthcare providers, hospital facilities, and other healthcare entities.
- Track and monitor provider orders for outpatient services, ensuring timely and accurate communication.
- Attach results and consult reports for provider review, facilitating informed decision-making.
- Improve patient compliance through proactive communication, written correspondence, and the patient portal.
- Assist patients with lost or misplaced orders, ensuring timely resolution and minimizing delays.
- Notify ordering providers of patient non-compliance, promoting timely intervention and care coordination.
- Facilitate follow-up appointments as needed, ensuring seamless care transitions.
- Engage in ongoing training and professional development, staying up-to-date on best practices and industry developments.
- Communicate effectively with colleagues, promoting a collaborative and supportive work environment.
- Make recommendations to improve departmental efficiency, contributing to the organization's continuous improvement.
- Work effectively under pressure, prioritizing tasks and managing competing demands.
- Cooperate with colleagues to cover staff members and provide support as needed.
- Ensure compliance with all company policies, procedures, and guidelines, upholding the organization's standards and values.
- Undertake special projects and administrative duties as needed, contributing to the organization's overall efficiency and effectiveness.
- Communicate concerns and patient issues with the Director of Nursing and/or Risk/Claims Manager, ensuring timely resolution and follow-up.
At least one year of experience as a Certified Medical Assistant or equivalent combination of education and experience. Strong interpersonal and communication skills, with the ability to work effectively in a fast-paced environment. Fluency in English and Spanish, with the ability to communicate with patients and providers in both languages.
