Position Summary

As a Support Care Specialist, you will play a vital role in assisting individuals in achieving independence and enhancing their quality of life. Your responsibilities will encompass a wide range of support services tailored to meet the unique needs of each participant.

Key Responsibilities:


In this position, you will be expected to perform the following essential functions to ensure the safety, care, and well-being of the individuals served by WellLife Network:


  • Facilitate activities that promote social responsibility, independence, dignity, and personal growth for each participant.
  • Implement individualized goals aimed at maximizing independence, including areas such as budgeting, personal grooming, nutrition planning, public transportation usage, community resource engagement, shopping, and housekeeping.
  • Complete necessary training and certifications in medication administration, crisis intervention, CPR, and First Aid, with employment contingent upon successful completion of all required exams.
  • Coordinate emergency evacuation procedures and ensure safety during drills and actual emergencies.

Essential Functions:
These functions include the ability to assist individuals in emergency situations, which may involve both verbal and physical support, as well as providing first aid when necessary.

  • Administer medications or oversee self-administration as a certified professional, ensuring accurate documentation in the electronic medication administration system where applicable.
  • Manage crisis situations and adhere to incident reporting protocols.
  • Monitor and document participant progress through daily checklists, behavior contracts, and progress notes, ensuring thorough communication with the management team.
  • Engage with participants regularly to review progress, identify needs, and update program plans, while assisting in accessing appropriate community resources.
  • Support participants in planning and attending recreational activities, contributing to their overall well-being through moderate physical engagement.
  • Assist in organizing daily activities and events tailored to participant interests.
  • Provide training in activities of daily living (ADL) and offer comprehensive support for individuals requiring full assistance, including personal hygiene and mobility support.
  • Participate in all scheduled staff meetings and mandatory training sessions to enhance professional development.
  • Serve as a positive role model for participants in various settings.
  • Maintain confidentiality and adhere to all privacy standards at all times.
  • Be adaptable to changes in work schedules and locations based on participant needs and program requirements.
  • Perform additional duties as assigned.

Qualifications:
A high school diploma or equivalent is required. Previous experience working with adults with intellectual or developmental disabilities is preferred.
