About the Role

Care Design NY is seeking a highly skilled Care Manager to join our team. As a Care Manager, you will play a critical role in helping individuals with intellectual and/or developmental disabilities achieve their long-term and short-term goals.

Key Responsibilities
  • Advocate for and with members to ensure informed decision making, informed consent, and appropriate guardianship.
  • Schedule, lead, and collaborate with members and their interdisciplinary team to conduct meetings and assessments, ensuring the development of a comprehensive, person-centered Life Plan that reflects the person's needs and desired life goals.
  • Implement, update, and monitor Life Plans, and facilitate individualized Life Plan reviews and approval processes.
  • Ensure integration of all needed and preferred supports and services, including medical, behavioral, social, habilitation, dental, psychosocial, and community-based, and facility-based long-term supports and services.
  • Maintain ongoing contact with critical people in a member's life.
  • Ensure timely submission of all documentation, including Life Plans, Progress notes, etc.
  • Assist members with maintaining benefits such as Social Security, Supplemental Security Income, Medicaid, and Medicare coverage, and Food Stamps.
  • Assist members to resolve problems in living, such as housing, utilities, the judicial system, and general safety.
  • Report abuse or neglect immediately when observed or reported.
Requirements
  • A Bachelor's degree with two (2) years of relevant experience, OR a License as a Registered Nurse with two years of relevant experience, which can include any employment experience and is not limited to case management/service coordination duties, OR a Master's degree with one year of relevant experience.
  • Strong communication skills, including verbal and written communication skills, along with strong interpersonal and organizational skills.
  • Excellent organizational, interpersonal, and verbal and written communication skills required.
  • Bilingual in English and Spanish.
