Coordinated Entry Specialist

Job Summary:
We are seeking a highly motivated and compassionate Coordinated Entry Advocate to join our team at Community Action Partnership of Lancaster County inc. As a Coordinated Entry Advocate, you will play a critical role in providing supportive services to survivors of domestic violence, connecting them with safe and affordable housing options, and advocating on their behalf to address systemic barriers to housing access.
Key Responsibilities:
- Perform Coordinated Entry/CHART assessments for survivors at DVS, identifying their housing needs and developing individualized service plans.
- Refer survivors to crisis/emergency, transitional, and permanent housing options, ensuring they receive the support they need to achieve stability and self-sufficiency.
- Provide trauma-informed safety planning and supportive services, including referrals, to survivors, promoting their well-being and empowerment.
- Collaborate with the Housing Team to coordinate rental assistance and provide ongoing support to survivors receiving rental assistance in permanent housing.
- Advocate on behalf of survivors with the Continuum of Care (CoC), countywide Coordinated Entry team members, and related housing partners, communicating their housing needs and challenges.
Requirements:
- Degree in the social services or related field, plus 2-5 years' experience working in the housing sector, or equivalent combination of education and experience.
- Bilingual skills (English and Spanish) are strongly desired.
- Proficiency with Microsoft Office programs required.
- Ability to maintain confidentiality of protected client and program information.
- Social justice mindset and strong desire to participate in and promote a trauma-informed, empowering culture that celebrates diversity.
