Role Overview:

The Clinical Care Transition Educator plays a pivotal role in delivering comprehensive clinical education and training to facilitate the seamless transition of patients into our innovative care delivery model.


Key Responsibilities:
  • Assess, instruct, and guide patients, caregivers, and healthcare facility staff on the integration of Option Care Health services and products, ensuring a smooth transition to our care model.
  • Conduct thorough evaluations to determine patient eligibility for our care delivery system.
  • Collaborate closely with the sales team to identify educational needs and implement strategies that enhance patient transition rates.
  • Uphold the confidentiality of patient information and adhere to legal standards for protecting sensitive data.
  • Document educational activities accurately using our proprietary applications and technology.

Supervisory Role:

This position does not entail supervisory responsibilities.


Qualifications:
  • Active and unrestricted Registered Nurse (RN) license is mandatory.
  • A minimum of two years of experience in the healthcare sector is required.

Essential Skills:
  • Proven ability to establish and maintain professional relationships across various organizational levels.
  • Knowledge of industry standards and practices relevant to healthcare services.
  • Experience in delivering high-quality customer service and evaluating client satisfaction.
  • Proficient in basic computer skills, including internet navigation and Microsoft Office applications.
  • Ability to identify operational challenges and propose effective solutions.
  • Access to reliable transportation for local travel to healthcare facilities.
  • Willingness to obtain nursing licensure in additional states as needed.
  • Strong organizational and presentation skills.

Travel Requirements:

This role requires 100% local travel to partner facilities, community hospitals, and medical offices to provide education and support.


Preferred Qualifications:
  • Experience in home or alternate site infusion services.
  • Proven track record in developing partnerships with service providers.

Salary will be determined based on the applicant's qualifications and experience.
