Position Overview:


The Clinical Care Transition Educator plays a vital role in delivering clinical education and training, ensuring a seamless transition for patients into our care delivery model.


This position collaborates closely with the sales team to enhance the transition of patients to our services.


Key Responsibilities:

  • Assess, instruct, and guide patients, caregivers, and facility personnel on the implementation of our services in various settings, ensuring a smooth transition to our care model.
  • Conduct thorough evaluations to determine patient eligibility for our care delivery model.
  • Collaborate with the sales team to identify educational needs and develop strategies to boost patient transition rates.
  • Uphold the confidentiality of patient information and adhere to legal standards for safeguarding sensitive data.
  • Document educational activities accurately using our technology and applications.
Supervisory Responsibilities:

This role does not include supervisory duties.

Qualifications:

  • Must possess an active and unrestricted Registered Nurse (RN) license.
  • A minimum of two years of experience in the healthcare sector is required.
Essential Skills:

  • Proven ability to build and maintain relationships across various organizational levels.
  • Familiarity with industry standards and practices relevant to our services.
  • Experience in delivering high-quality customer service and evaluating satisfaction.
  • Proficient in basic computer skills, including internet, email, and Microsoft Office applications.
  • Capability to identify operational challenges and propose effective solutions.
  • Access to reliable transportation for local travel to various facilities.
  • Willingness to obtain nursing licensure in additional states as needed.
  • Strong organizational and presentation skills.
Travel Requirements:


This position involves 100% local travel to partner facilities, community hospitals, and medical offices for educational support and patient transitions.

Preferred Qualifications:

  • Experience in home or alternate site infusion services.
  • Background in developing partnerships with service providers.
Compensation:

Salary will be determined based on the applicant's qualifications and experience, with a minimum pay starting at $39.98.
