Position Overview:


The Clinical Care Transition Educator plays a vital role in delivering clinical education and training to facilitate the smooth transition of patients into our care delivery model.


This position collaborates closely with the sales team to enhance the number of patients successfully transitioned to our services.


Key Responsibilities:

  • Assess, educate, and train patients, caregivers, and facility personnel on the integration of our services and products in alternative settings, ensuring a seamless transition to our care model.
  • Conduct thorough patient evaluations to determine their suitability for our care delivery system.
  • Collaborate with the sales team to identify educational needs and implement strategies to boost patient transition rates.
  • Uphold the confidentiality of patient and proprietary information, adhering to legal standards for safeguarding sensitive data.
  • Document educational activities accurately using our applications and technology.
Supervisory Responsibilities:

This role does not include supervisory duties.

Qualifications:

  • Active and unrestricted Registered Nurse (RN) license is mandatory.
  • A minimum of two years of experience in the healthcare sector is required.
Preferred Skills:

  • Proven ability to build and maintain relationships across various organizational levels and with external partners.
  • Familiarity with industry standards and practices relevant to our services.
  • Experience in delivering high-quality customer service and evaluating satisfaction levels.
  • Proficient in basic computer skills, including internet, email, and Microsoft Office applications.
  • Ability to identify operational challenges and propose effective solutions.
  • Access to reliable transportation for travel to care facilities and patient homes.
  • Willingness to obtain nursing licensure in additional states as needed.
  • Strong organizational and presentation skills.
Travel Requirements:


This position requires 100% local travel to various facilities, hospitals, and medical offices to promote our services and provide educational support.

Compensation:


Salary will be determined based on the applicant's qualifications, experience, and alignment with market data.
