Job Summary

MGA Homecare is seeking a skilled Pediatric Transitional Care Nurse to join our team. As a key member of our care coordination team, you will play a vital role in ensuring a seamless transition of care for pediatric patients being discharged from the hospital to their homes.

Key Responsibilities
  • Transition of Care: Provide nursing care and assist in transitioning patients from hospital to home, ensuring a smooth and safe transition.
  • Care Coordination: Assist with hospitals and agency to facilitate hospital discharge and prevent re-hospitalization, ensuring timely and effective care coordination.
  • Patient Education: Provide education to patient and family caregivers as identified, empowering them to take an active role in their care.
  • Team Collaboration: Collaborate with the patient care team to ensure comprehensive care and support for patients and their families.
  • Training and Development: Train new permanent nurses when identified, ensuring continuity of care and expertise.
  • Office Support: Assist with office clinical tasks as needed, supporting the overall operations of MGA Homecare.
Requirements
  • Education: Current state license as a Registered Nurse, BSN preferred.
  • Experience: Minimum of two years of nursing experience in a Home Health or Acute setting, with Trach and Vent experience and NICU/PICU experience preferred but not required.
  • Certifications: Current CPR card and Current health certificate as applicable.
  • Transportation: Current state driver's license, reliable transportation, and automobile insurance.
