Role Overview:
The Clinical Care Transition Educator plays a pivotal role in delivering educational support and training to facilitate the seamless transition of patients into our innovative care delivery model.
Key Responsibilities:
- Assess and educate patients, caregivers, and healthcare staff on the integration of Option Care Health services in various settings, ensuring a smooth transition.
- Conduct thorough evaluations to determine patient eligibility for our care delivery model.
- Collaborate with the sales team to identify educational needs and implement strategies to enhance patient transition rates.
- Safeguard patient confidentiality and adhere to legal standards regarding sensitive information.
- Document educational interactions accurately using company technology and applications.
This position does not include supervisory responsibilities.
Qualifications:
- Active and unrestricted Registered Nurse (RN) license is mandatory.
- A minimum of two years of experience in the healthcare sector is required.
- Proven ability to build and maintain relationships across various organizational levels.
- Familiarity with industry standards and practices relevant to our services.
- Strong customer service orientation with a focus on quality and satisfaction.
- Proficient in basic computer skills, including Microsoft Office applications.
- Experience in identifying and resolving operational challenges.
- Access to reliable transportation for local travel to various healthcare facilities.
This role requires 100% local travel to partner facilities, community hospitals, and medical offices for education and training purposes.
Compensation:
Salary will be determined based on the applicant's qualifications and experience, with a minimum pay starting at $39.98.
